Grant Requirements
- As a condition of receiving an award, recipients are required to submit both a Mid-Year Evaluation and a Final Evaluation documenting successful outcomes of the project.
- Each grant must include either a pre- and post-test or a pre- and post-survey. These documents must be attached with your Mid-Year and Final Evaluations.
- Evaluations must include:
- A report documenting final results, based on the goals and objectives outlined in the proposal.
- A final budget analysis.
- Recipients must submit all receipts documenting grant-funded purchases of materials, supplies, and services.
Additional Guidelines
- All awards are granted on a competitive basis. For evaluation purposes, do not include your name or your school name in the body of the proposal.
- Some items, materials and activities are NOT fundable. Please consult the expenditures section for allowable funding categories.
- All grants must have measurable outcomes supported by academic data.
- All purchases of individual assets costing $500.00 or more must be tagged, inventoried annually by the Foundation, and returned to the Foundation for proper disposal if no longer in use.
Technology Requests
- If you are requesting iPads, Chromebooks, Smart Boards, or other technology, all quotes must go through the OCSD Helpdesk at OCSDHD@okaloosaschools.com.
- Be sure to include any network support fees for the current school year.
- Applicants should clearly note that the quote is for a Teacher Grant with the Okaloosa Public Schools Foundation.
- This ensures the work order is properly generated and tracked.
- All technology requests must include a plan for future network support in years two and three (Seat/Support Management, if required). All quotes must be submitted with your application.
Important Notes
- Do not include sales tax. Schools are tax-exempt.
